Found the vampire mobs this morning and got infected, ran through some of the quest as well. wonder if I shouldn't have told others about the mobs after everyone tried to intentionally tried to kill them and screw over others (everyone wants to be first) but I think a couple got it at least.

Also, there is a lot of speculation on this, but let me try and talk about what I know and have an idea about

 

Known

1. you can get infected at low levels. I was infected with lycanthrope at level 9, cured it, and then got vampirism at 15

2. The spawns for vampires are "Bloodfiend" and for werewolves are "Werewolf"

3. The quest will start after you get the infection if you go to a major town (one with its own unique map) and it does not have an active event going on that has displaced NPCs. Rune for example has a daedric invasion

4. the alter for the quest is in the end level zone, reapers march for dominion near the north of the map

5. the quest does not scale down, so you will need to be able to take out level 41 mobs

6. no one can help you in the instance, you have to be able to kill the mobs on your own. but you can kite, which took me about an hour. recommended to build up your ranged attacks

 

Hypothesis (untested, guesses from beta testing, and patterns we've seen)

1. werewolves spawn on waxing moon and full moon. Vampires spawn on waning moons and new moon

2. there is a 1/3 chance for vampire/werewolf/none

3. mobs will spawn by themselves or near mobs with a solo spawn, so the other mobs won't overwhelm the players

4. there is a 1 in a thousand chance to be infected by a non-named vampire or werewolf by letting them hit you

5. the rare spawns can appear anywhere in the level end level zone, including dungeons and caves

6. the rare mobs can spawn in PVP
